Model Release Forms
These model release forms give me permission to take and use these people for photos to be published within my magazine. This form contains their consent for me to use their images in production of my magazine and to be published online or within this blog. The Top of my model release forms provides the name and 'logo' which identifies which magazine and production company model are signing too. My Company is called 'SOUNDZ'
